What this is
This is a short series of questions about what you observe day to day — how they are getting on, what you have noticed, and what concerns you, if anything. It takes around eight to ten minutes.
Completely private — from both sides
Your answers are completely separate from theirs. They will not see what you say, and you will not see their answers. What you share goes to the health team alongside their own report — not as a judgement, but as a fuller picture that helps services understand exactly how to support them to stay well and at home.
Everyone wants the same thing
You, the person you care for, and the health team all want the same outcome: for them to be happy, well, and at home on their own terms. Your observations help make that more possible.

Your right to a Carer's Assessment
Under the Care Act 2014
As a carer, you have a legal right to an assessment of your own needs from Kent County Council Adult Social Care — regardless of the needs of the person you care for. This assessment can lead to support, respite, and practical help for you.
Register as a carer with your GP
Free and straightforward
Telling your GP you are a carer means they can factor this into your own health care and make sure you are getting any support you are entitled to. Many GP surgeries have a designated carer lead.

When you are ready, you can send your observations to the health team. Your responses will go alongside the person's own report — not as a comparison, but as a fuller picture to help them provide the right support.

What happens next: Your responses will be reviewed by the Folkestone and Hythe Neighbourhood Health Service frailty team, who will use both reports to plan how best to support the person you care for.
